我國全民健康保險自1995年開辦之後，不但深深影響民眾的就醫行為，也改變我國的醫療結構甚鉅，尤其在實施總額預算制度、DRGs制度、浮動點值計算之後，對於地區醫院的經營更是雪上加霜，使得台灣醫療院所的變化呈現兩極化的趨勢，因此思考醫院轉型或是優化經營策略是每個地區醫院的重要課題。本研究為質性的個案研究，以在醫療院所四個層級中皆有醫療機構的大型財團法人中的地區醫院為研究對象，透過PEST分析與SWOT分析醫院現況，並結合人口老化趨勢、政府長期照顧議題與當地醫療現況，討論其經營困境，例如新競爭醫院加入、無大型業外收入、硬體空間不足等。並針對個案研究醫院提出經營策略建議，例如聚焦地區醫院、結合政府長照政策、院際策略聯盟、加強體系整合、發展特色自費醫療項目等等，希望可以做為其他地區醫院在競爭策略上的參考，以達成永續經營之目的。

Taiwan’s medical structures have changed dramatically since the establishment of National Health Insurance program in 1995. The structural changes have not only inflicted on the district hospitals especially after the implementation of total budget system, diagnosis related groups system and floating point-value system, but also affected people’s utilization of health care resources. Hence, healthcare reforms and optimization of care services seem to be a topic of concern. Using a qualitative case study approach, we assessed the impact of the ageing population, long-term health care policies and quality of local health care services on the operation of a district hospital in northern Taiwan. In addition, challenges such as competition from other hospitals, lack of non-operating income and insufficient hardware system were also examined. We suggested that the case study hospital should be more district-hospital-focused, combine government long-term healthcare policies into their operational system, increase strategic alliances, strengthen the core of management system, and develop self-pay health-related services. The proposed strategies may serve as a reference for other district hospitals, as well as achieving sustainable management.
