本研究主要探討資深諮商師處理個案自殺行為之經驗探討,包括面對自殺行為個案帶來的衝擊、因應、長期影響及有效與無效介入等經驗歷程,以質性研究方法來進行。透過五位進入諮商領域超過10年的資深諮商師進行深度訪談,結果如下:
一、資深諮商師面對自殺個案帶來的衝擊及其內涵:
(一)面對個案自殺死亡的衝擊經驗:包括難過與生氣、遺憾、與個案家屬接觸的無助與焦慮、驚嚇、壓力及質疑自己的專業能力。
(二)面對自殺企圖個案的衝擊經驗:生氣、恐懼、焦慮、擔心、疲憊、無助、無力、罪惡感、被拋棄、面對倫理的困境及沉重的壓力。
二、資深諮商師面對自殺個案的調適方法:有專業支持系統及個人支持系統二類。前者包括督導、參加專業訓練、參加研討會或個案研討會、跟精神科醫師討論與接受治療等五項;後者包括與專業同儕討論、與機構同事討論、閱讀、宗教上力量及家人的支持等五項。
三、資深諮商師處理自殺個案帶來的長期影響及其內涵:專業的正向影響有四項,共十二個因子。分別為對諮商師專業角色的澄清與理解、增加對個案自殺行為的了解、增加對個案自殺行為的處理能力、及其它;個人正向影響有三項,包括:對生命及死亡的體會、促進諮商師的學習動力、改變對人生的態度;個人的負向影響有二項,包括:身體狀況不佳、家人的誤解或無法體會。
四、整體而言,資深諮商師處理自殺個案之有效的介入與處理,包括正確的同理、傾聽與主動介入、傳達不放棄、接住與承戴、建立資源、重新檢視個案的資源、教育媒體、處理被個案影響的高危險群;無效的介入,包括表面的勸告、太快通報家人、技術的僵化與膚淺、感受到諮商師的害怕、對個案資源與環境作錯誤的評估。
本研究最後針對上述結果加以討論,並據此結果針對新手諮商師或其他助人工作者及未來研究者提出具體建議。
The purpose of this study was to understand how senior counselors cope with suicidal clients, the impact on them, their reactions, effect as well as effective and ineffective interventions. The participants of this study were five senior professional counselors; research data were collected from in-depth interview with these counselors who had dedicated to this field for more than ten years. The main findings are as follows:
The impact on two counselors experienced client successful suicide included feeling sad, angry, regretful, helpless, anxious, frightened; stressed and self-blamed; five counselors who experienced suicide attempts clients faced the dilemma of ethics and huge stress; anger, feeling fearful, anxious, nervous austed, worried; helpless, guilty, and abandoned. Those counselors used both professional support and personal support system to cope with this crisis.
The long-term effects on senior counselors who had dealt with suicide clients included four profession effect, three personal positive effects, and two negative ones. Finally general findings senior counselors’ effective and ineffective interventions and treatments when coping with suicide clients were examined.
The implicaton, discussion as well as future suggestions for counseling practice and research are addressed.
