許多不專業的健康資訊來源可能存在偏見和不確定性。如果沒有良好的資訊,消費者可能無法有效選擇最優的服務,滿足他們的健康需求。
這項研究報告主要研究民眾對於健康資訊的需求,和其如何使用健康資訊,以選擇或更換就診醫師。研究群組樣本消費者選擇在台北都會區,有其豐富的健康服務資源、成熟的健康保險、以及完整的運輸系統,民眾得以方便獲得健康服務。研究設計是橫斷面群組研究,採用訪談式結構化問卷調查法,包括醫院外科專科的51名患者和另外51名前往私人外科診所求診的患者。
總共102位受訪者的分析發現,他們經常尋求非專業健康資訊,也許是向家人,朋友,或是曾經讀過的媒體健康訊息。研究結果發現較多女性(比男性),以及較多年紀小於六十歲(比大於六十歲)的民眾在就醫前尋求親友及專業的意見。人生滿意度的訪問結果則與現代醫療的利用無明顯關聯。屬於健康結果的健康生活品質明顯回饋影響民眾的進一步就醫行為。其中的一般健康狀況、生理職能、生理機能與軀體疼痛與看病次數有明顯關聯。從生理面向摘要也可看出生理層面的困擾與看病次數相關。健康生活品質的另一方面的心理層面影響結果就不太一樣,自覺活力較好的受訪者與安排健康檢查有明顯關聯,但是情感職能受到限制的受訪者比較退縮而減少就醫。健康生活品質的社交功能比較差的受訪者傾向受到媒體資訊的影響而更換就診醫師。
整體而言,受訪者經常利用非專業健康資訊,縱使他們知道其內容不確定性,約有一半依此選擇醫師,約四分之一依此更換醫師。人口學特徵、健康資訊的需求、和健康意識的因素相互影響,屬於健康結果的健康生活品質並且明顯回饋影響健康行為。這些因素交互作用也改變民眾尋找最佳的健康服務的偏好。
本研究這些問題的答案有其重要性。如果國民健康保險要鼓勵消費者使用有效的健康資訊來源,並降低他們使用不確定資訊所產生不必要的尋求或就醫行為,必須提供公正、公開、及方便取得的健康資訊,並且改變誘因,以誘導民眾利用方便、專業的健康資訊。
Bias and uncertainty are common on various unprofessional health information sources. Without good information on their choices, consumers may be unable to approach optimal services that fulfill their health needs.
This study reports on the needs and use of information on selecting/switching physician by consumers in Taipei, a metropolitan region long known for its abundant health-care services as well as mature health insurances and transportation systems that support peoples‘ convenient approach to health-care plans. Research design was cross-sectional cohorts study, interviewing using a structured questionnaire survey that encompass 51 patients approaching to cardiovascular specialty clinics in a hospital and the other 51 to a private surgical clinic.
Many of the 102 respondents reported their search for advice from other resources, perhaps by taking to family, friends, or by surfing medical columns. A half of them actually selected physicians and a quarter ever switched physicians base on those uncertain information. Factors of demographic characteristics, health consciousness, and health-related quality of life interacted each other and affecting peoples’ behavior in searching for optimal health-care services.
The answers to these questions can be important if health insurance programs want to encourage consumers to use effective health information sources and discourage them from unnecessary hospital-shopping on uncertain information.
