心肌梗塞為冠狀動脈硬化所導致的缺氧性心肌組織壞死,由於成人心肌組織缺乏再生的能力,因此壞死的心肌組織可能會嚴重影響心室的功能。文獻上,曾有研究群利用骨髓裡分離得到的間葉幹細胞(Mesenchymal stem cells, MSCs),注射至受損的心肌組織中,來改善心室的功能。然而,MSC在體外培養皿增生後,往往需要利用如trypsin之類的酵素脫附成單顆懸浮的細胞,以利經由針筒的心肌注射。這項酵素脫附細胞的動作,往往會破壞細胞與細胞之間的連結性、細胞外間質,甚至傷害了細胞本身的存活及增生的能力等,因此可能影響後續細胞移植的治療效果。另外文獻上有報導,在注射利用酵素處理後的懸浮MSC (dissociated MSCs)的過程中,會有大量流失的現象,且植入的MSC過於分散於心肌組織中,嚴重的限制了其治療的效果。本篇目的為利用實驗室先前所發發的甲基纖維素溫度敏感成膠特性,製備一細胞完全不貼附於其上的細胞培養皿。這項技術將用來大量生產MSC細胞團塊(Spheroids),用於心肌梗塞病患的注射治療,以克服dissociated MSC心肌注射所遇到的問題。結果顯示,注射Spheroids的實驗組三個月後其左心室功能明顯優於對照組(dissociated MSC)。在病理組織切片分析中,注射Spheroids的實驗組其三個月後心肌梗塞的範圍較小。此外注射Spheroids的實驗組其梗塞區域心室壁與其心室壁的彈性皆明顯改善,進而避免心室持續擴大。在血管新生的免疫染色結果中發現注射Spheroids可以刺激受損組織的血管新生,且新生血管數量比注射dissociated MSC的對照組明顯增高。以上結果證明,利用甲基纖維素水膠培養Spheroids應用於急性心肌梗塞的治療,能增進移植後的細胞數量,並改善其心肌梗塞後的左心室功能。
