摘要
近年來,乳癌對於台灣已成為一種高發生率高死亡率的癌症,因此乳癌對於女性的生命威脅是不可忽視的。為了把握乳癌患者的治療黃金時間,必須利用高解析度的乳房X光攝影作為檢查的工具。但乳房X光機為一個提供診斷資訊的放射線設備,所以除了要得到一個高品質以供診斷價值之外的影像外,還須注意受檢者所接受到的輻射劑量問題。希望在維持影像品質的先決條件下,受檢者的劑量能夠盡可能地降低。
本篇論文經由全國大規模的調查,針對乳房X光攝影之輻射劑量進行分析,並建立台灣乳房X光攝影之劑量指引水平,以提升醫療曝露品質保證和輻射防護。另外,從我國整體國民健康的角度,乳癌篩檢之受檢婦女的輻射劑量必須加以評估,所以針對乳房X光攝影劑量有關的臨床條件做大量的收集和分析。調查樣本是由3家醫學中心所收集,樣本數量非常大,故所獲得的條件參數可以代表我國婦女的平均值,未來只需將我國乳房X光檢查受檢頻率考慮進去,即可評估台灣婦女接受乳房X光攝影的集體劑量,進而評估輻射引起的癌症風險,以做為國家制定公共政策及規範的參考。
經由普查分析後,我國平均乳腺劑量範圍為0.39 ~ 2.68 mGy,平均值為1.42±0.42 mGy。並取四分之三位數1.62 mGy,做我國乳房X光攝影之劑量指引水平。乳房X光攝影集體劑量部分,訂定台灣乳房X光攝影之分齡參考乳房與分齡參考曝露因子,並分析出各年齡層CC view與MLO view之平均乳腺劑量。未來只需將分乳房X光攝影之分齡檢查頻率考慮進去,即可得到我國乳房X光攝影之集體劑量,進而評估輻射引起的健康風險。
